Munson Army Health Center offers primary care to active-duty service members, Family members and retirees. Primary care is first-contact care for persons with any undiagnosed sign, symptom, or health concern. Primary care includes health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis and treatment of acute and chronic illnesses.All appointments for Primary Care are scheduled through our Appointment Line. Munson offers virtual and face-to-face appointments. Meet our providers. Some services may need a referral from the patient's Primary Care Manager.

  • Blood Pressure Checks are done on the 3rd floor. You must have a doctor's order in your records to have a walk-in check. Patients can arrive at the 3rd floor front desk anytime from 7:30 a.m.-2 p.m. (except for the lunch hour 11:30 a.m.-1 p.m.) Please tell the front desk clerk that your provider requested for you to get a blood pressure check. The front desk clerk will look into yourelectronic medical record (MHS GENESIS) for the order. The team nurse will come out and bring you back to the exam room to perform a blood pressure check. It will then be recorded inthe electronic medical records for your doctor to see it.
  • School physicals required for the 2024 -2025 school year will be offered later this summer. If you are PCSing to Fort Leavenworth, we recommend that you get your child's school physicalbefore arriving.

Patient Centered Medical Home Model

Army Medicine is transforming from a healthcare system to a System for Health beginning with our primary care services. We are transitioning to comprehensive, collaborative care.

You, the patient, are our center, our focus, our partner in your healthcare journey. Patient Centered Medical Home is a multidisciplinary approach to deliver comprehensive primary care.
Each patient will partner with a team of healthcare providers – physicians, nurses, behavioral health professionals, pharmacists, and othersto develop a comprehensive, personal healthcare plan.

The Team guides and navigates the health care system with you. YOU, the patient takes an active role as an engaged decision maker.Patients have an active role in their health care and have an opportunity to meet with the health care team on a regular basis which builds trust and strengthens communication.

At its heart, Patient Centered Medical Home is healthcare the way it should be – easy to access, patient-centered, team based and quality focused on comprehensive coordinated care.

Who is on Your Care Team?

  • Primary Care Manager (PCM)—is the individual you selected or was assigned to you when you registered with Munson for your care. This individual may be a Medical Doctor (MD), a Physician Assistant (PA), or a Nurse Practitioner (NP). You may change your provider by calling TRICARE at 1-844-866-WEST (9378) or at www.tricarewest.com.
  • Licensed Practical Nurses (LPNs)—and/or Medics who are assigned to work specifically with your PCM.
  • Registered Nurse (RN)—person assigned to work specifically with your PCM.
  • Other Specialists—persons who work with all teams and are available to provide guidance regarding your care such as a Clinical Pharmacist, a Nutritionist, and Case Manager.
  • YOU—the Patient, who is at the center of this TEAM. PCMH is a multidisciplinary approach to delivering primary care and coordinating care both in and outside the facility. You can assist your team in providing the best possible care by communicating your health history, current medications and by practicing self-management goals.

  • Provide you safe and appropriate healthcare services that are accessible to you.
  • Work with other healthcare Specialists, in providing your care.
  • Refer you for specialty care when necessary and coordinate with the network providers as needed.
  • Answer healthcare questions.
  • Readiness is our #1 priority. Readiness is driven by your health status. Your team is here to assist you with your choices.

  • Take an active part in your healthcare and participate in decisions regarding your health.
  • Become an informed and effective healthcare consumer using information and skills provided to you by your team to improve your health.
  • Address impact of conditions on your lifestyle.
  • Communicate with your care team about your health and concerns you may have.
  • Discuss any issues or problems you have in trying to comply with your care plans.
  • Take responsibility for your own healthcare and ask questions if you do not understand whether it ismedication management, exercise routine, orwhat you are supposed to do before surgery.
